To make an appointment, you can contact the office by phone at 613-673-4318 or online if you are registered with the Health Myself Portal
When you make an appointment, choose the type of appointment the most suitable for your health needs. We categorized the appointments in three types:
Acute appointment/Follow-up (15 minutes): Acute is the one you take when you do not feel well. Examples include; respiratory infections, sudden and acute pain, fever, etc. Follow-up is a visit to follow up on a health issue previously addressed with your doctor or at your hospital discharge. These appointments are available to be booked online if you are registered with the Health Myself Portal (choose urgent appointments or regular visits)
Preventive/Annual visit (30 minutes): these include comprehensive reviews that will allow for immunization, and referral for screening such as a mammogram, colonoscopy and bone densitometry. It is also a good time to discuss general health questions and concerns. These appointments are available to be booked online if you are registered with the Health Myself Portal (choose Male or Female physical)
In order to book the proper appointment and time needed, you will need to give the receptionist some idea for the reason for your appointment.
PREPARING FOR YOUR APPOINTMENT
To help ensure that your experience is positive, the Plantagenet Family Health Team offers the following tips to prepare for your appointment.
Please have the tests that have been ordered done a few days before your appointment so that results are available to be discussed.
Asking your provider if you need a test before the next visit, and obtaining the requisition.
Asking a family member, friend or peer to come to the clinic with you to help you remember the information discussed.
CAN A FAMILY MEMBER OBTAIN PERSONAL INFORMATION ON MY BEHALF OR BOOK AN APPOINTMENT FOR ME?

The Plantagenet Family Health Team has grown over the past years with new patients and staff. It is not possible any longer to know all the patients and what their privacy needs are.
It has been a common practice for many years, that a spouse or partner will make arrangements for medical appointments or inquires if a test results have arrived in the office. This has been done on a verbal – trust relationship.
After reviewing the privacy practice, it has been necessary to change this practice.
As of now, in order for us to discuss information about your health with another party (eg. spouse, caregiver, family member), written consent must be provided. We also require authorization to disclose or transfer medical records.
Parents and guardians of children under the age of 16 years do not need to complete this process unless there are special circumstances such as:
A court order pertaining to custody.
The children do not have the same surname as the parent or guardian and it would be difficult to determine the relationship.
For the purpose of privacy, children 16 years and older are treated as adults and have the right of complete privacy. If they wish to include their parents or guardian, they will need to complete the Privacy Policy:
Staff will only communicate directly with patients.
It is impossible to communicate with parents or guardians of children 16 years and older if there is no written consent on file.
If written consent is on file, it is possible to communicate with the delegate.
Authorizing changes:
You may complete the Privacy Authorization Change form that is on the Plantagenet Family Health Team web site and forward it to the office. This form is for a single delegate.
APPOINTMENT TIME
It is important that you arrive a few minutes (10-15 minutes) before your appointment so that we have time to gather important health information or complete necessary preparation procedures prior to your appointment.
REVIEW OF SYMPTOMS
Be prepared to discuss your symptoms with your health care provider. Write down all of the issues you would like to discuss during your visit. Begin with the most bothersome issue. This will allow your provider to get a clear picture upfront of what your major concerns are and appropriately attempt to address issues in the time allowed.
ITEMS TO BRING TO YOUR APPOINTMENT
Please bring your OHIP card with you so that we can update your registration. Both adults and children should also bring their immunization record so that it can be verified and updated when required.
It is best to bring your bottles of medications if possible along with a list of your medications, including the name and dose of each one or you can obtain a current medication list from your pharmacist. You will also need to bring information on any vitamins/dietary supplements and over the counter medications that you take.
Bringing any logbook or record that you keep, such as blood pressure, blood sugar, weight or food diary.
Also, do not forget to bring your glasses if you need to fill out forms, and bring any specialized equipment you use (shoes, orthotics, braces etc …) to help our healthcare professionals better assess your condition during the examination.
LATE ARRIVAL
It is important that you arrive a few minutes (10-15 minutes) before your appointment so that we have time to gather important health information or complete necessary preparation procedures prior to your appointment. If a patient is more than 10 minutes late for an appointment, the appointment may need to be rescheduled. This is to ensure that the patients who arrive on time do not wait longer than necessary to see the provider. You may be given the option to wait for another appointment time on the same day if one is available. We will try to accommodate late‐comers as best as possible, but cannot compromise on the quality and timely care provided to our other patients.
PAYING FOR MISSED APPOINTMENTS OR LAST MINUTE CANCELLATIONS
It’s important to know that when you miss an appointment or cancel at the last minute, you not only impact your doctor, but you impact other patients who could have taken your appointment. To the extent that you can, it’s important to make sure you go to the appointments you’ve made or cancel them with enough notice that another patient can take your spot.
Your doctor can charge you a reasonable fee ($30.00) if you miss an appointment or cancel with less than 24 hours’ notice. If you have legitimate reasons for missing or canceling your appointment last minute, tell your doctor.
SPECIALIST APPOINTMENT
It may take several weeks to receive a call for an appointment at a specialist clinic after your doctor has requested it. Ask your doctor what is the waiting time for the specialty in question.
